A heated debate is unfolding in the world of pharmaceuticals, with a controversial plan to import cheaper drugs from Canada to Florida at its heart. This idea, proposed by Florida Governor Ron DeSantis, aims to tackle the high cost of pharmaceuticals in the state by tapping into Canada's more affordable medication prices. However, the plan has hit a snag, with Canadian drug manufacturers and distributors denying any involvement.
Despite Florida's insistence that the program is underway, industry representatives in Canada have clarified that they are not collaborating on this initiative. The average U.S. prices for pharmaceuticals are significantly higher than in Canada, with industry data suggesting a threefold difference. Canadian manufacturers already export a substantial amount of pharmaceutical products to the U.S., and further exporting at Canadian prices would undercut their existing U.S. market rates.
In 2024, the U.S. Food and Drug Administration (FDA) approved Florida's drug importation plan, but only for a limited period of two years. Yet, Politico recently reported that no drugs have been shipped through this program, despite Florida's investment of over US$80 million in a warehouse for the initiative. This facility has since been repurposed to store emergency equipment for hurricane season.
Molly Best, press secretary for Mr. DeSantis, confirmed to The Globe and Mail that the state is proceeding with the drug importation program. She stated that Florida's vendor has been actively negotiating with Canadian manufacturers and distribution partners to import eligible prescription drugs into the U.S. However, associations representing Canadian drug manufacturers and distributors have refuted these claims, stating that they are not involved in these discussions.
Erin Polka, vice-president of communications for Innovative Medicines Canada, which represents patented drug makers, confirmed that they have not spoken to Florida about this program, nor have their members, to their knowledge. Angelique Berg, CEO of the Canadian Association for Pharmacy Distribution Management, echoed this, stating that they are unaware of any distributors in talks with Florida.
Berg further explained that Canadian drug prices are lower due to the public healthcare system and the unique cost-control measures employed by Canadian regulators. She emphasized that changing the shipping pathways for these products would not address the root cause of high U.S. prices.
"Quite frankly, the entire idea is bewildering," she remarked.
Health Canada has also warned drug companies, wholesalers, and distributors against exporting pharmaceuticals outside the country if it could lead to domestic shortages. This warning was reiterated when The Globe contacted the department on Wednesday.
Florida's plan also contradicts other initiatives from the Trump administration aimed at addressing pharmaceutical pricing and manufacturing. Throughout 2025, U.S. President Donald Trump frequently threatened tariffs on pharmaceuticals, including launching a national security investigation into the drug supply chain in April and announcing 100% tariffs on branded drugs made by companies without a U.S. manufacturing presence in September. However, these threatened levies never materialized, and if imposed, could result in higher prices for patients as manufacturers pass on the costs.
In recent months, the Trump administration has shifted its focus to signing "most favored nation" agreements with drug makers, where they agree to lower U.S. medication prices in exchange for better reimbursement through Medicare insurance plans.
